Online Retreval Certificate Application Form

       Please enter the eleven digit (11) number given at the time the application was entered online.
       Then click on the 'Submit Number' button at the bottom of the screen.

       The system will automatically locate the application whether it is for a Birth, Death or Marriage.

       An application not found, is reported as an invalid number.

       To clear the input field after an error is made, click the 'Clear Content Form' button.
